The updated portal now allows online generation of health cards using mobile or ration card details.

Check eligibility using your ration card number or mobile number on the official PMJAY portal.
आधिकारिक पीएमजेएवाई पोर्टल पर राशन कार्ड नंबर या मोबाइल नंबर से अपनी पात्रता जांचें।
Eligible families are identified via SECC 2011 data, with state-specific updates.

Generate Your Health Card | स्वास्थ्य कार्ड कैसे जनरेट करें

Visit the Ayushman Bharat/PMJAY portal and go to “Print Health Card” or “Generate Card”.
आयुष्मान भारत/पीएमजेएवाई पोर्टल पर “Print Health Card” या “Generate Card” पर जाएं।
Enter your eligible ID — ration card or mobile number — and click “Search”.
अपना योग्य पहचान — राशन कार्ड या मोबाइल नंबर — दर्ज करें और “Search” पर क्लिक करें।
If found eligible, click “Generate Card” and download or print it immediately.
